There are three main methods of administering testosterone and what is most important is the preference for the patient. Many of our men prefer a pellet to be inserted over other methods for the convenience it offers. Luteinizing hormone is one of the hormones your brain produces to tell the testicales to produce testosterone and sperm. It is normal for the production of LH to diminish severlay when on external testosterone as the brain is saying “we are good and don’t need to produce any more testosterone.” As a results LH levels drop dramatically. HCG is an analog to LH meaning it functions like LH and administration of it can keep the testicles working and keep the natural production going.
There is much controversy that exists whether one needs to keep natural production going. We have yet to see one case of a man that was unable to recover his natural production after stopping testosterone therapy. It’s our opinion that is is unnecessary in most cases but this will be discussed between the patients and the clinicians. Even is a man plans on having kids there are plenty of methods of getting his sperm count to the level necessary to impregnate.
This is completely dependent on how the individual reacts. Having prolonged levels of too much estrogen in a man increases the incidence of cardiovascular events. The use of a medication like Arimidex to bring the levels down to ideal levels may be warranted. This is a case by case situation that is evaluated closely as too low levels of estrogen can also be very problematic for a man.
Being on testosterone will shut down the production of hormones that are necessary for keeping a healthy sperm count. If nothing is done about this then impregnation can be very difficult. There are many medications that can aid in recovery the ability to produce sperm again and we have yet to see a case where a man was permanently damaged and unable to conceive from being on Testosterone.
